A HRD (Human Resources Development) form that must be completed by an employer whenever an interruption of earnings occurs for any employee.

To calculate how long a person can collect benefits, it is only necessary to determine how long a person worked in the previous 52-week period (as noted in the Record of Employment provided by the employer) and the regional unemployment rate (which is published monthly by Statistics Canada and can be obtained from any Canada Employment Centre office).
