Hertwig's epithelial root sheath initiates the formation of dentin in the root of a tooth by causing the differentiation of odontoblasts from the dental papilla. El epitelio dental interno y externo, proliferan desde el borde cervical del órgano dental formando una doble capa de células conocida como vaina epitelial de Hertwig. Esta estructura crece alrededor de la papila, entre la papila y el folículo, hasta que rodea todo menos la porción basal de la papila. El extremo de la vaina radicular, el diafragma epitelial, encierra el foramen apical primario. A medida que las células epiteliales internas de la vaina radicular encierran progresivamente más y más a la papila dental en su expansión, se inicia la diferenciación de los odontoblastos a partir de la células de la periferia de la papila dental.
