La metadona es un opioide, es decir, un narcótico sintético de acción parecida a los opiáceos(morfina, etcétera), pero que no deriva del opio. Los adictos a la heroína son dependientes de opiáceos y sufren los síntomas de la abstinencia si la concentración de esta droga en sangre baja de cierto nivel. Una dosis apropiada de metadona reduce los síntomas de abstinencia.
