OBS

RDEB is an autosomal recessive inherited condition. This means both parents are carriers, yet they are unaffected. When each parent has a copy of the altered gene, there is a 1 in 4 chance or 25% that the child will be affected.

OBS

Although in some cases this form of EB [epidermolysis bullosa] can be mild with generalized blistering, typically the recessive forms of EB tend to be more severe. Onset is usually at birth with areas of missing skin. Generalized blistering then scarring can occur on skin surfaces and mucous membranes. Scarring may limit range of motion of extremities. Fusion of fingers and toes and contractures cause deformity and loss of function.

OBS

There is usually generalized blistering noted at birth. Blistering may be generalized or appear only on the hands, feet, elbows or knees : this is usually due to mechanical trauma. Rarely does scarring cause immobility and deformity of the hands and feet. Small cysts or milia are seen at sites of scarring. There may be mild involvement of the mucous membranes, nails may be thick, dystrophic or destroyed.

Cervical flexion deformity(chin on chest deformity) is a complication of severe ankylosing spondylitis. This fixed posture of the cervical spine can cause significant difficulties with hygiene, nutrition, and forward gaze. Patients experience inability to open their mouth wide enough to maintain adequate nutrition, hygiene problems such as skin ulcers on the chin or chest, and problems with forward gaze leading to difficulties with ambulation. Psychosocial aspects of this deformity are also considerable. Surgical intervention includes posterior C[ cervical] 7–T[ thoracic] 1 extension osteotomy/fusion with sterile midprocedure halo placement. This procedure involves significant manipulation of the cervical spine, with possible catastrophic consequences.

Kyphoplasty is a newer treatment for patients immobilized by the painful vertebral body compression fractures associated with osteoporosis. Like vertebroplasty, kyphoplasty is a minimally invasive procedure that can alleviate up to 90% of the pain caused by compression fractures. In addition to relieving pain, kyphoplasty can also stabilize the fracture, restore height, and reduce deformity.... Kyphoplasty is performed under local or general anesthesia. Using image guidance x-rays, two small incisions are made and a probe is placed into the vertebral space where the fracture is located. The bone is drilled and a balloon, called a bone tamp, is inserted on each side. These balloons are then inflated with contrast medium(to be seen using image guidance x-rays) until they expand to the desired height and removed. The balloon does not remain in the patient. It simply creates a cavity for the cement and also helps expand the compressed bone. The spaces created by the balloons are then filled with PMMA [poly(methyl methacrylate) ], the same orthopaedic cement used in vertebroplasty, binding the fracture. The cement hardens quickly, providing strength and stability to the vertebra, restoring height, and relieving pain.

Cette opération consiste en : - une excision elliptique transversale de la peau, sur une largeur d'environ 2 à 3 mm, sur la face dorsale de l'articulation I.P.D.; - l'excision du cal tendineux à ce même niveau, allant jusqu'à la capsule qu'on évite d'ouvrir, ce qui n'est pas toujours possible; - quatre fils monofilament de nylon 3-0 sont passés à travers la peau, le tendon, la capsule, d'un côté puis de l'autre; ils ne sont pas immédiatement noués, mais en croisant les fils, on teste que l'extension complète de l'articulation peut être obtenue.
